UConn Field Education Form | School of Social Work

The field education department will use the information collected in this form to support your placement for the 2022-2023 Academic Year. The form should take 5-10 minutes to fully complete and we ask that you attach your resume in the final screen. When complete it will automatically send to the field education department and a copy will be sent to you. Once received by the field education department; your file will be assigned to a field coordinator. The field coordinators work on folders in the order in which they are received by the office. One of the coordinators will reach out to you to schedule a time to discuss your field form and to plan for the placement process. It is an expectation that you regularly check your e-mail throughout this process. Should you have questions or have not heard please reach out to sswfielded@uconn.edu

FIELD EDUCATION HOURS & TIME FRAME(Required)

BSW 14 Hours a week (September-April)

MSW 15 Hours a week (Sept. – June)

MSW 20 Hours a week (September-April)

MSW Summer Block 2nd year IGFP students only (May-August)

Field Education is a school year long commitment starting in September and ending in April. Based on the program; students must meet a minimum number of hours. Please indicate how you plan on completing the field expectation for the 2022-2023 year. Please note the following: 2nd Year MSW students must complete 20 hours a week to have degree conferral in May. 2nd year MSW students that are Fifteen hours a week will have a degree conferred in August. Students must indicate their intention to apply for summer block by December 10, 2021. If there is not sufficient interest, we will not accept any applications for summer block. Employed Social Work Program(Required)

UConn allows for currently employed students to earn field hours through an Employed Social Work Placement (ESW). The placement must follow the general field education requirements as described throughout the Field Education Manual. A written agreement describing the arrangements must be developed between the School, student and agency. Approval is contingent upon the soundness of the educational plan. DCF Stipend Program(Required)

The DCF Stipend program is a program open to students who are not employees of DCF. All students must apply for this program and if accepted agree to apply for employment with DCF upon graduation and if accepted work for the agency for 2 years. Are you applying for the DCF Stipend Program?

DCF Cohort Program:(Required)

This program is open to DCF employees only. This is a 5-semester program. The cohort begins classes during the spring semester and enters field in the fall. DCF employees must apply for the GES program at DCF for the first year of field education and the advanced year of field education will be in DCF. Are you applying for the DCF Cohort Program?
